- Oral glutathione (capsules, tablets) has historically been criticised for poor gastrointestinal absorption. A 2014 study in the European Journal of Nutrition found that single-dose oral glutathione (1000mg) raised plasma GSH levels by only 30–35% above baseline within 90 minutes, with levels returning to baseline by 6 hours. However, liposomal and sublingual formulations demonstrate superior bioavailability. Liposomal encapsulation protects the tripeptide from gastric degradation, achieving plasma increases of 60–80% in some trials.
- Injectable glutathione (IV or intramuscular) bypasses first-pass metabolism entirely, delivering 100% bioavailability. A 2020 systematic review in Dermatologic Therapy analysed 14 studies on IV glutathione for skin lightening: doses ranged from 600mg to 1200mg administered 1–2 times weekly for 8–12 weeks. Melanin index reductions averaged 18–26%, comparable to high-dose oral protocols but achieved in shorter timeframes. The trade-off: IV administration requires clinical oversight, sterile technique, and incurs significantly higher cost (often $150–300 per infusion).
- Topical glutathione formulations face a penetration barrier. The tripeptide's molecular weight (307 Da) and hydrophilic nature limit dermal absorption through the stratum corneum. A 2018 study in the Journal of Cosmetic Dermatology tested 2% topical glutathione cream applied twice daily for 10 weeks. Melanin index reduction was 4.7%, statistically insignificant compared to placebo. Topical formulations work best as adjunctive therapy when combined with penetration enhancers (niacinamide, alpha-arbutin) or used alongside oral supplementation.
- For research applications, oral reduced L-glutathione at 500–1000mg daily remains the most studied and cost-effective route.
